Obtaining Copies of Your Records from Calais Regional Hospital
If you need to obtain copies of your or your minor children(s) health records from Calais Regional Hospital, hospital policy states that we must have a minimum of one week (7 business days) notice to respond to the request.
Please note: Maine law allows hospitals furnishing medical reports or treatment records to the patient or the patient’s authorized representative, to charge for these copies. Charges may not exceed $10.00 for the first page and $0.35 for each additional page.
If you need copies of medical records, please call Calais Regional Hospital’s Health Information Department at 454-9314 to schedule the release of your information. The days and hours of the Health Information Department are Monday-Friday, 8:00 a.m. – 4:00 p.m.
